Congenital Diaphragmatic Herna through Foramen of Bochdalek

Abstract
Congenital diaphragmatic hernia through foramen of Bochdalek, posterolateral diaphragmatic hernia, is
the result of a congenital malformation of the posterolateral region of the diaphragm.
This hernia is the most common type of Congenital diaphragmatic hernia requiring surgical emergency
in the newborn and infant, the reason why the most of the cases accmompany the herniation of the intra
abdominal viscera into the thoracic cavity combining cardiopulmonary distress.
The anomalies are approximately 5 times as common on the left side as on the right. In most case
no sac exists, but rarely the peritoneum or pleura may be present to form a hernial sac but very thin.
The purpose of this paper is to comment on the history embryologic maldevelopment and pathogenesis
of this disease throughout revewing literature and to describe 2 clinical cases which were treated
surgically by authos, in department of general surgery in Chungnam Medical Center during last year.
The defect of diaphragm in both of them located on the left without hernial sac and associated no
other abnormalities. One is 4 months old male infant and the other is 7 months old female infant.
Surgical operation with reduction of the intestines into the abdomen and closure of the defect in the
deaphragm were performed through abdominal approach as an emergency. Postoperative course were
relatively uneventful, and both of them was discharged without any other complication except would
disruption which was easily cured with secondary closure in 4 months old male infant.
The collapsed lungs were readily relieved with introducing chest tube at the operation because of there
were neither hypoplasis nor agencies of the ipsilateral lung.
